Membership Fee's
The monthly fee covers the doctor's time at flat rate.
One-time enrollment fee is $50.00/person,
not to exceed $100.00 per family.
Age Fee
0 - 20 with adult membership........$20/month
0 - 20 w/o adult membership........$55/month
21 and over................................$85/month
Other Membership Fee's
Home Visit* (within service area)....Free!
Home Visit* (outside service area).... $100/visit + travel time - please discuss with Dr. Lucero
*Home Visits are reserved for acute or chronically home bound patients only.
You can cancel your membership at anytime, however there is a re-enrollment fee of $200/person
- Applicable Sales Tax will be added to all service charges.
- Prices are subject to change, however members will be notified in advance.
- Preferred methods of payment: cash, check, automatic bank transfer (bring routing # and bank account #)
- FSA/HSA and Credit/Debit Cards are accepted, but there will be a 4% convenience fee.
